Originally, in the Mirage Comics, she was a high-ranking Foot Clan leader and Shredder's adopted daughter. Her loyalty shifted depending on her sense of honor, sometimes aligning with the Turtles against Shredder's more ruthless strategies.

Subsequent adaptations, such as the 2003 animated series, expanded upon this, portraying her as skilled in combat and possessing a strong sense of duty to the Foot Clan, although conflicted by her growing respect for the Turtles.

In the 2012 animated series, her origin was significantly altered. Here, Karai is revealed to be Miwa, Hamato Yoshi/Splinter's biological daughter, who was kidnapped and raised by Shredder, believing him to be her father. This version showcases her internal struggle as she learns the truth and must choose between loyalty to the Foot Clan and her biological family.
